FHN now has two of five Health Centers that have undergone dramatic performance improvement and are operating under the new FHN Model of Care. Team-based care is a key piece to the success of this model of care, and all care teams have adapted and implemented best practices that are supporting optimum efficiency. FHN is seeing overall transformation of care to a patient- centric medical home that is producing consistent positive outcomes! Implementation to the remaining Health Centers will continue through 2024.
