  • Established in 1972, Family Health Network of Central New York, Inc. (FHN) is a federally funded, not-for-profit, community health center, serving Cortland and contiguous counties.
  • FHN is the only source of care in three of the communities served, and the only source of primary care sliding fee adjustments for qualifying individuals and families in the Cortland County region.
  • In 2022, FHN provided 44,795 medical, dental, behavioral health, and school-based visits to 14,612 people.
  • 25% of the patients served are 100% and below the federal poverty level, while 22% of the patients served are over 200% of the federal poverty level.
  • 41% of the patients are on Medicaid; over 5% are uninsured/under-insured.
  • During the 2022-2023 school year, FHN offered pediatric and adolescent services to over 1,100 students enrolled in FHN’s School Based Health Program at four rural schools, with no out of pocket expenses to the child’s family.
  • FHN’s OB Providers delivered 69% of the babies born at Guthrie Cortland Medical Center in 2022.
  • The FHN Occupational Health Program has contracts with over 113 local businesses, providing pre-employment physicals and drug screening, random drug screening and injury assessments and treatment.
  • Family Health Network is proud to accept private insurance, Medicaid, Medicare, and provides sliding fee adjustments to those who are eligible.
  • FHN employs a Certified Applications Counselor who provides assistance in insurance enrollment, free of charge, for all individuals – regardless if they are a patient of Family Health Network.
  • Cortland County is a designated Health Professional Shortage Area (HPSA) for the low-income population. This designation denotes areas having shortages of primary care, dental care, or mental health providers based upon statistics of the community. FHN’s current HPSA designation for medical, dental, and mental health are respectively: 20, 24, and 22.
