Family Health Network is partnering with Coleman Associates to implement an operational transformation that will focus on team-based care and the patient experience.  As an organization, we are privileged to play a vital role in the health and wellness of the communities we serve.  Everyone deserves great healthcare that is in time and on time.  Through this collaborative, our patients and staff deserve to have their time respected while providing and receiving efficient quality care.  Over the course of the next year, we will focus on reducing our cycle time to 35 minutes or less, completing 75% of charts by the end of the clinic day (with 100% completed within 48 hours), and having no more than 2 missed opportunities, per care team, per day, to ensure the greatest possible access and experience for our patients, while reducing staff burnout.
